ParaMed seeks a Registered Nurse for scheduled community visits in Wawa, Hawk Junction, and Dubreuilville. You will support patients of all ages with differing care complexities, helping them remain comfortable at home. The work includes individualized assessments, care plan monitoring, collaborative patient engagement, and professional record updates.

Key Responsibilities:

- Assess individualized patient needs using appropriate tools
- Execute and continuously monitor patient care plans
- Build rapport and trust through team-oriented engagement
- Maintain professional records related to patient encounters
- Update records related to patient care plans

Requirements:

- Register with the CNO as a Registered Nurse
- Maintain good standing with the CNO
- Hold a valid driver’s license and vehicle access
- Travel to patient residences within designated areas
- Provide required after hours on call coverage
