CarePartners seeks community nurses for home and clinic care, offering versatile scheduling while requiring full nursing scope and travel readiness.

Registered Nurses and Registered Practical Nurses provide services in home, clinic, retirement home, or school settings. The work includes in-person and virtual patient care, care planning, and interdisciplinary communication. Full-time and part-time schedules include weekdays, weekends, and on-call rotation.

Key Responsibilities:

- Develop and maintain effective Community Health Services care plans
- Provide exceptional nursing services in person and virtually
- Serve patients in homes, clinics, retirement homes, or schools
- Communicate with patients, families, and interdisciplinary care teams




- Perform other clinical duties as required

Requirements:

- Registered Nurse or Registered Practical Nurse degree or diploma
- Current College of Nurses of Ontario registration in good standing
- Valid driver’s license and vehicle for regular travel
- Excellent English verbal and written communication skills
- CPR certification, immunization record, and required background checks
