Community nursing Registered Nurses assess patients and monitor care plans during scheduled visits across diverse home and community settings. ParaMed’s full-time Registered Nurse role supports patients of all ages and varying care complexities. You will build therapeutic relationships while helping patients remain comfortable at home. The position requires assessment, collaborative communication, documentation, and ongoing care-plan monitoring.

Key Responsibilities
Assess individualized patient needs using appropriate tools and techniques Execute and continuously monitor individualized care plans Build rapport and trust through collaborative patient engagement Maintain and update professional patient encounter records Travel to patient residences within a designated area

Requirements:
Current CNO registration as a Registered Nurse in positive standing Valid driver’s license and access to a vehicle Community care experience considered an asset Reliability, flexibility, and willingness to travel Availability for required after-hours on-call coverage Bring assessment, care planning, and relationship-building skills to ParaMed’s community nursing services supporting patients at home. #J-18808-Ljbffr
