Deliver person-centered nursing care at AgeCare Lakeview in Vancouver, using clinical judgment, medication management, and care coordination skills. This regular full-time role supports residents’ dignity.

Reporting to the Director of Care, the Licensed Practical Nurse provides goal-directed care within provincial standards. You will work with an interdisciplinary team, maintain assessments and documentation, administer treatments, and coordinate resident care. Experience in senior or long-term care is preferred.

Key Responsibilities:

- Conduct resident assessments and maintain accurate care documentation
- Implement, evaluate, and adjust individualized care plans
- Administer medications and treatments under applicable regulations
- Coordinate transitions, conferences, schedules,



and service providers
- Respond to changing conditions and medical emergencies

Requirements:

- Current provincial or state Licensed Practical Nurse registration
- Robust clinical knowledge and care coordination skills
- Current CPR and First Aid certification
- Electronic documentation and Microsoft Office proficiency
- Current Vulnerable Sector Check or willingness to obtain
