Provide specialized geriatric care with Foundation Health through comprehensive assessments, complex condition management, and interdisciplinary collaboration. This part time position may evolve to full time.

As a Geriatrician, you will support older adults within Foundation Health’s team-based specialized care program. The work focuses on medical, cognitive, and functional issues, patient independence, and quality of life. You will collaborate with nurses, home care professionals, dietitians, and physiotherapists.

Key Responsibilities:

- Conduct comprehensive geriatric assessments for older adult patients
- Diagnose and manage complex medical, cognitive, and functional issues
- Develop and implement individualized treatment plans




- Collaborate with multidisciplinary healthcare professionals
- Educate and support patients, families, and caregivers

Requirements:

- MD degree with Geriatric Medicine specialization
- RCPSC certification in Geriatric Medicine or equivalent
- Licensure with the CPSO
- Robust clinical and communication skills
- Interdisciplinary healthcare experience is an asset
