This Registered Nurse or Registered Psychiatric Nurse works with the North Shore Intensive Case Management Team. The position provides triage, versatile outreach, counseling, and integrated care coordination. Practice supports clients with mental illness, substance use, and chronic medical conditions through interdisciplinary collaboration.

Key Responsibilities:

- Conduct mental and physical status assessments
- Assess client needs and prioritize care requirements
- Provide counseling and crisis intervention
- Develop and adjust integrated client care plans
- Facilitate connections across acute and community services

Requirements:





- Current BCCNM registration as a Registered Nurse or Registered Psychiatric Nurse
- Two years’ recent related mental health and substance use experience
- Valid British Columbia driver’s license and personal vehicle access
- Knowledge of psychiatric assessments, psychopharmacology, and substance use treatment
- Ability to communicate and collaborate within interdisciplinary teams
