Provide advanced clinical care at Mill Creek Care Centre in Barrie, using autonomous assessment, diagnosis, treatment, and monitoring. Collaborate with residents, families, and care teams.

This Nurse Practitioner reports to the Director of Care and is accountable to the Medical Director. The position supports integrated care planning, chronic disease management, wound care, palliative care, and antimicrobial stewardship. You will coach clinical staff, contribute to education and research, and advance evidence-based practice initiatives.

Key Responsibilities:

- Deliver comprehensive primary care using a person-centred approach
- Complete assessments, medication reconciliation, care conferences, and care planning
- Prescribe, deprescribe, and monitor medications and treatments
- Coordinate referrals,



diagnostics, results review, and follow-up
- Lead quality improvement, staff education, and transfer avoidance initiatives

Requirements:

- Current CNO registration as Nurse Practitioner RN(EC)
- Ontario RN(EC) eligibility for applicants registered elsewhere
- Excellent interpersonal and communication skills
- Long-term care experience preferred
- Current screening, health testing, vaccines, and two references
