ParaMed’s home health care services need a Registered Nurse skilled in assessment and patient records. This part time position serves patients where they call home.

As a community Registered Nurse, you will care for patients of all ages with differing complexity of needs. Scheduled visits help patients remain comfortable at home while you execute and monitor individualized care plans. You will work within the County Area, Erie St. Clair District, and travel between residences as required.

Key Responsibilities:

- Care for patients during scheduled community nursing visits
- Assess needs across wide-ranging patient ages and care complexity
- Continuously monitor and execute individualized care plans




- Engage patients collaboratively while building rapport and trust
- Document encounters and plans in professional patient records

Requirements:

- Registered Nurse registration with the CNO in good standing
- Valid driver’s license with access to a vehicle
- Willingness to travel to patient residences
- Required after hours on call coverage
- Community care experience is an asset
