ParaMed seeks a full-time Clinical UCP Supervisor to oversee PSWs and HSWs serving community clients. You will assess, implement, and evaluate clinical interventions while supporting secure, quality, ethical care. The position includes collaboration with nursing team members, patients, families, and community staff.

Key Responsibilities:

- Supervise PSWs and HSWs across community care assignments
- Investigate patient safety incidents and document required action plans
- Support clinical documentation and funder reporting submissions
- Provide education, coaching, mentoring, and health teaching
- Lead huddles, meetings, and individual staff discussions

Requirements:

- Current unrestricted RN or RPN registration in Ontario
- Minimum five years practicing as an RN or RPN
- Comprehensive patient health assessment skills
- Community nursing principles and clinical documentation knowledge
- Valid driver’s license and reliable transportation
