Bayshore Health Care in Montreal is seeking a Case Manager to coordinate patient journeys from welcome calls through discontinuation of therapy. You will work with physicians, DANs, nurses and patients, ensuring timely access to treatment and ongoing support in a fast-paced, team-driven environment. The role requires bilingual English/French communication, solid customer service, and solid data-entry skills.
