This Registered Practical Nurse facilitates assessments and procedures through approved virtual technologies. The position connects clients with specialists and clinics while supporting care coordination, referral triage, and tele-diagnostic examinations. You will also complete administrative tasks within EMRs.
Key Responsibilities:
- Facilitate health assessments and procedures using relevant protocols
- Connect clients with specialists through approved virtual technologies
- Coordinate care with referring providers and clinical teams
- Triage referrals and complete required specialist documentation
- Support examinations using tele-diagnostic equipment when required
Requirements:
- Valid registration with the College of Nurses of Ontario
- Minimum three years clinical experience in a healthcare setting
- Familiarity with multiple health specialties and pathophysiology
- Team-oriented experience with physicians and allied healthcare professionals
- Valid driver's license and reliable, insured vehicle
