This Registered Practical Nurse role focuses on client-specific care grounded in best evidence and practice. You will assess needs, create and review care plans, and support transitions by connecting clients with local services. Designated primary providers remain accountable for assigned caseloads while collaborating across care teams.

Key Responsibilities:

- Assess clients comprehensively and identify unique care needs
- Build care plans using best evidence and practices
- Implement and evaluate planned client care

Requirements:

- College of Nurses of Ontario registration
- Passion for community care and evidence-informed practice
- Current CPR certification
- Driver's license, vehicle, and travel availability
- Willingness to attain required Saint Elizabeth certifications
