This ongoing Diabetes Specialist Nurse chance sits within the Countywide Diabetes Service. The Band 6/7 post provides specialist diabetes care in a nurse-led environment. You will monitor clinical needs, educate patients, and promote independence through evidence-informed self-care support.

- Provide specialist diabetes assessment and management
