Vancouver Coastal Health seeks a Community Health Nurse for the Home Health Team. You will support clients with acute, chronic, palliative, or rehabilitative needs through client and family centred care. The work includes care planning, direct care, service coordination, and transitions.

Key Responsibilities:
Assess clients and determine service eligibility
Develop and adapt individualized care plans
Coordinate nursing and other client services
Deliver direct care in community settings
Support clients navigating available services

Requirements:
Current Registered Nurse registration with BCCNM
One year recent related clinical experience
Experience supporting complex chronic health conditions
Valid British Columbia Driver's License
Knowledge of community health nursing practice
