This Primary Care Physician role offers full-time and part time work at WellFort sites, including Bramalea and Four Corners. Physicians address social determinants of health while diagnosing and treating acute and chronic conditions. Responsibilities also include advocacy, client education, and partner collaboration.

Key Responsibilities:

- Deliver comprehensive care emphasizing accessibility and prevention
- Consider social and economic determinants during assessments
- Advocate with clients to reduce healthcare access barriers
- Collaborate with staff, providers, and community agencies




- Support medical protocols and nurse practitioner consultations

Requirements:

- Experience or openness serving communities facing healthcare barriers
- Critical analysis of health determinants and social factors
- Excellent written, oral, and interpersonal communication skills
- Ability to work independently within interdisciplinary teams
- Language skills relevant to the organization’s community are an asset
