You will assess needs, support care planning, facilitate transitions, and coordinate with providers, pharmacies, payers, and internal partners. Requirements:
Bachelor’s degree in nursing or related field
Equivalent experience may satisfy educational requirements
Four or more years in care management

Relevant certification preferred
