Canadian Mental Health Association Muskoka–Parry Sound seeks a Case Manager for its Community Outreach and Support Team. The work includes intensive case management, psychosocial rehabilitation, crisis intervention, and treatment. Under clinical supervision, you will assess, plan, coordinate, and provide rehabilitation and support services for clients with severe and persistent mental illness.

Key Responsibilities:
Carry out intensive case management for clients
Provide psychosocial rehabilitation and crisis intervention
Assess client needs and plan appropriate services

Requirements:
Bachelor’s degree in relevant human services or health science
One year supporting people with mental illness preferred
Experience in counselling, case management, or crisis intervention
Indigenous Cultural Awareness and Education
Valid Ontario G or G2 license and appropriate insurance
