At Sentrex Health Solutions, the Field Nurse Case Manager coordinates Patient Support Program services for patients, physicians, and healthcare providers. You will manage registration, enrolment, follow-up, therapy support, and communication with healthcare partners. The work includes patient and provider education, case management protocols, data entry, and adverse event reporting.

Key Responsibilities:
Coordinate patient support services according to program protocols
Register patients and healthcare providers, completing required follow-ups
Provide education on diseases, therapies, and disease management




Monitor adherence, therapy changes, clinical information, and adverse events
Liaise with physicians, pharmacies, and infusion service providers

Requirements:
Registered Nurse in valuable standing with the provincial nursing college
Two to four years of clinical experience
Bilingual English and French communication skills
Experience in oncology, gastroenterology, or rheumatology
Patient Support Program experience is an asset
