Support patients at home across Amherstburg as a Community Visiting Nurse, using assessment skills, care plans, and cooperative engagement. The schedule is part time.
ParaMed is seeking a School Nurse and Community Visiting Nurse for school and home settings. During weekdays in the school year, you will care for pediatric patients with differing complexity of needs. During breaks, home visits serve patients of all ages, helping them remain comfortable at home.
Key Responsibilities: Deliver community home visits during school breaks
Assess patients using appropriate tools and techniques
Execute and monitor individualized care plans continuously
Engage patients collaboratively while building rapport and trust
Maintain accurate records of encounters and care plans
Requirements: Registered Practical Nurse registration with the CNO
Good standing with the College of Nurses of Ontario
Valid driver’s license and vehicle access required
Willingness to travel between designated patient residences
Required on-call coverage, including after hours
Use your nursing assessment, care planning, and relationship-building skills to support ParaMed patients throughout the Amherstburg community.
