Support acute care patients as a casual Social Worker at Toronto General Hospital, using assessment and discharge planning expertise. Work weekday and weekend programs.
UHN seeks a Social Worker for inpatient acute care programs within Allied Health and the Hyperbaric Chamber. Working with the Manager and interdisciplinary team, you will assess patients, develop treatment plans, and support discharge planning. The position includes intervention with patients, families, significant others, and qualified colleagues.
Key Responsibilities: Assess patients and formulate recommendations or treatment plans
Plan discharges for inpatient acute care patients
Implement and modify intervention plans as required




Liaise with community agencies and interdisciplinary team members
Provide clinical teaching and participate in relevant research
Requirements: Master’s degree in Social Work required
Ontario College registration required
Acute care experience required
Three years’ hospital interdisciplinary experience preferred
Excellent communication and problem-solving skills
Bring acute care social work, patient assessment, intervention planning, and discharge planning expertise to UHN’s Toronto General Hospital.
