Reporting to the Director of Care, this LPN delivers planned, patient-oriented, goal-directed nursing care. You will collaborate within an inter-professional team, maintain current assessments and plans, and communicate changes. Care follows provincial standards, Resident Care requirements, and AgeCare policies.
Key Responsibilities: Collaborate within the inter-professional team
Maintain current assessments, plans, and clinical documentation
Communicate resident care changes as required
Schedule and participate in resident care conferences
Evaluate care plan effectiveness and adjust accordingly
Requirements: Practical Nurse Diploma
Current registration in the applicable province
Membership in good standing
Three or more years supporting seniors’ health
Leadership, coaching, and mentorship skills
Use your seniors’ health experience, leadership, and care planning expertise to support AgeCare residents and families with dignity.
