Support geriatric and frail seniors at West Park Healthcare Centre as a Registered Nurse, using comprehensive assessment and rehabilitation nursing expertise.
UHN seeks a permanent full-time Registered Nurse for the Geriatric Functional Enhancement Service and Frail Seniors Transition Home. You will coordinate nursing care within an inter-qualified, primary nursing model, supporting health promotion, treatment, and optimal function. Days and nights are required.
Key Responsibilities: Plan and coordinate comprehensive nursing care for patients and families
Implement care using clinical knowledge and team input
Educate patients, families, facilities, and community providers
Participate in rounds, care planning, and family conferences




Support students and new staff through mentorship and supervision
Requirements: Diploma in Nursing; baccalaureate degree preferred
Good standing with the College of Nurses of Ontario
Experience with geriatric and frail seniors populations
Computer literacy in word processing and Electronic Patient Record
Effective communication, problem-solving, and decision-making abilities
Bring geriatric care, inter-professional collaboration, and rehabilitation nursing skills to UHN’s West Park team.
