Reporting to the Manager of Transitional Services, this Registered Nurse supports patients transitioning home or back into their communities. The position uses data, technology, effective communication, and shared decision-making to inform patient- and family-centred care plans. Interprofessional collaboration and clinical expertise guide each patient’s progress.
Key Responsibilities: Assess, plan, implement, direct, and evaluate nursing care
Complete initial and regular health assessments
Manage, administer, and document prescribed medications




Apply dressings and treatments according to orders
Monitor symptoms, reactions, treatments, and patient changes
Requirements: Bachelor of Science in Nursing
Current registration with the Provincial Association of Registered Nurses
Valid CPR and First Aid Certification
Clear Criminal Record and Vulnerable Sector Checks
Demonstrated supervisory and employee evaluation skills
Bring transitional care, clinical assessment, medication management, and collaborative communication skills to Shannex’s West Bedford team.
