This ongoing Diabetes Specialist Nurse opportunity sits within the Countywide Diabetes Service. The Band 6/7 post provides specialist diabetes care in a nurse-led environment. You will monitor clinical needs, educate patients, and promote independence through evidence-informed self-care support.
Key Responsibilities: Provide specialist diabetes assessment and management
Monitor and adjust care plans following current guidelines
Promote patient independence and long-term self-care
Deliver education supporting healthier lifestyle management
Maintain detailed clinical records and professional standards
Requirements: Current NMC registration and strong nursing credentials
Significant experience delivering diabetes care
Ability to manage adult Type 2 diabetes patients
Clinical assessment and patient education proficiency
Ability to work independently within multidisciplinary teams
Bring diabetes care, care planning, and patient education skills to adult services within Peterborough’s Countywide Diabetes Service.
