Support adults transitioning home from hospital through occupational therapy, evidence-based interventions, and coordinated community care with VHA Home HealthCare. This position supports As an Occupational Therapist, you will assess adults with higher needs and provide consultation, recommendations, and interventions. You will help clients reintegrate into home and community settings through a short-term program lasting up to 90 days. Collaboration with clients and integrated care teams supports safer discharges, greater independence, and coordinated schedules. Key Responsibilities:



Assess clients using professional practice guidelines and standards Implement interventions based on assessments and client input Coordinate care plans and intervention schedules with teams Document assessments, treatment plans, progress, and discharges Support safe care through continuous quality improvement Requirements: Degree in Occupational Therapy from a recognized school Good standing with the College of Occupational Therapists of Ontario Valid driver’s license with access to a vehicle Compassionate, empathetic, and self-driven approach Electronic documentation and virtual care skills Bring occupational therapy, caseload management, and team-oriented care skills to VHA’s adult transition program. #J-18808-Ljbffr
