Community nursing expertise meets scheduled patient visits as a Registered Practical Nurse with ParaMed, supported by Registered Nurses for complex care.
This full-time position serves patients of all ages in varied settings. You will help patients remain comfortable at home while practicing within your scope. The work includes assessment, care planning, therapeutic relationships, and professional documentation across designated communities.
Key Responsibilities: Complete scheduled community visits within your nursing scope
Provide ethical and protected nursing care
Assess individual patient needs using appropriate techniques
Execute, monitor, and adjust care plans
Maintain professional patient records after encounters
Requirements: Current CNO registration as a Registered Practical Nurse
Valid driver’s license and access to a vehicle
Willingness to travel between patient residences
Experience with vulnerable populations
Required after-hours on-call coverage
Apply community nursing, patient assessment, and care planning skills while supporting comfort and independence at home.
