You will support community clients through the Acute Care Transitions Program. The role emphasizes holistic care planning, qualified autonomy, and collaboration with interdisciplinary teams. Services address physical, cognitive, social, emotional, and spiritual client goals. Provide client-centered care in community settings Support clients transitioning from hospital to home Participate in joint care planning Address physical, cognitive, social, emotional, and spiritual needs Deliver accountable care and services to assigned clients Valid Personal Support Worker certificate, RN, or RPN Availability for days, evenings, and every other weekend Bring client-centered care, holistic planning, and community support skills to SE Health’s Acute Care Transitions Program. #
