This full-time contract role supports clients in housing departments through physical and psychiatric assessments, psychotropic injections, medication reconciliation, diabetes care, and coordinated appointments. Key Responsibilities:

- Conduct physical and psychiatric assessments and make appropriate referrals
- Organize, coordinate, and administer depot injections




- Review medication records and respond to medication errors
- Provide health teaching and medication management training
- Respond to urgent health concerns using harm reduction principles

Requirements:

- Diploma in Nursing as a Registered Practical Nurse
- Current College of Nurses of Ontario registration in good standing
- Valid driver's licence, vehicle access, and required insurance
- Current CPR Certificate
