Deliver direct client care in Long Term Care as a Health Care Aide, using communication and independent judgment across days and evenings.

Prairie Mountain Health’s Health Care Aide provides clinical support under established standards, policies, and team leader direction. Assist clients with daily living, personal hygiene, mobility, meals, elimination, and individualized care plans. Maintain dignity, confidentiality, independence, and culturally respectful service while documenting observations and care.

Key Responsibilities:

- Assist clients with activities of daily living
- Follow care plans and report client changes
- Document care, observations, intake, output, and measurements
- Provide hygiene, mobility, feeding, and elimination assistance




- Maintain infection prevention and environmental safety practices

Requirements:
