Bayshore Health Care is seeking an Occupational Therapist to join a home-based care team in Ontario. You will assess, treat, and educate clients to maximize independence as they transition from hospital to home, working with a multidisciplinary team.

You will document thoroughly, follow COTO guidelines, and support ongoing client outcomes in a shared client record. A driver’s license and reliable transportation are required.
