Key Responsibilities:

- Conduct home-based assessments for patients at falls risk
- Develop care plans and recommend falls-risk reduction strategies
- Provide patient and family education

- Participate in meetings, rounds, and quality improvement initiatives

Requirements:

- Recognized post-secondary Occupational Therapy program completion
- Current positive-standing Ontario occupational therapist registration
- Minimum one year of clinical rehabilitation experience

- Valid driver’s license and access to a car
