Nisbet Lodge is a 103 bed long term care home. We are currently accepting resumes for the Social Services Worker position. Reporting to the CEO, the Social Services Worker will coordinate resident admissions, discharges, financial administration, funding applications, and family relations. The ideal candidate will support residents and their families with care and professionalism while ensuring accurate documentation, regulatory compliance, and effective collaboration with internal teams, PGT, HCCSS, and community partners.

- Post room vacancies and process new resident application files for approval through HBG in collaboration with HCCSS.
- Provide tours of the building for potential residents.
- Coordinate resident admissions, transfers, and discharges, ensuring a smooth transition process by completing necessary legal financial documents.
- Track occupancy of the home and update admissions and discharges in PCC.
- Set up residents with recent PGT accounts and manage their financial appointments.
- Perform annual resident rate recalculations for subsidy purposes.
- Manage ERISSA
- Collaborate with PGT to arrange appointments, document transfers, correspondence for PIN money, and expenses.
- Conduct capacity assessment and follow up on invoices to PGT.




- Manage resident trust accounts and handle financial transactions in compliance with regulatory requirements.
- Host and maintain minutes of quarterly family meetings.
- Family/PGT/ODSP liaison
- Help residents and families to apply for funding.
- Resident liaison for crisis.
- Coordinate all residents’ billing information with the Business Manager and Manager of Finance.
- Health card Renewals for residents.
- Attend care conference and assist with follow up with the family members.
- Coordinate residents’ needs by arranging external services and appointments.
- Supporting residents with mental health and social needs.
- Work with CEO and DOC as requested.

Qualifications:

- Bachelors in Social Work or Social Services Worker
- Current registration and good standing with the Ontario college of social worker or Social services worker
- Minimum 2 years working in a long term care home setting
- Knowledge of the Fixing Long term care act, Residents Bill of Rights and related regulations
- Strong interpersonal and communication skills
- Ability to work as part of the interdisciplinary team
- Knowledge of community resources, healthcare systems and support services for seniors.

Pay: $32.00-$34.00 per hour

Expected hours: 37.5 per week
