HarmonyCrew Home Health Care is a growing home care provider dedicated to delivering compassionate, reliable, and personalized care to seniors and individuals in their homes.

We are seeking an experienced and motivated Client Care Coordinator to support client assessments, care coordination, community relationships, and business growth.

The ideal candidate will have experience in the home care industry, strong communication skills, and the ability to build meaningful relationships with clients, families, and community partners.

Key Responsibilities

- Conduct initial client consultations and in-home non-clinical assessments.
- Develop and coordinate personalized care plans.
- Respond to new client inquiries and support client onboarding.
- Build relationships with healthcare professionals, senior communities, and referral partners.
- Work with the Business Development team to attract and onboard new clients.
- Coordinate caregiver scheduling, assignments, and client services.
- Assist with caregiver recruitment and onboarding.
- Maintain client relationships and conduct regular follow-ups.
- Support daily operations and ensure quality client care.

Qualifications

- Minimum 2 years of experience in home care, healthcare coordination, or a related field preferred.
- Experience with client assessments, care planning, and client onboarding.
- Strong communication, organizational, and interpersonal skills.




- Experience in community outreach, client relations, or business development is an asset.
- HCA, LPN, RN, or related healthcare education is an asset.
- Knowledge of Surrey and surrounding communities is preferred.
- Valid BC driver's licence and reliable transportation.
