Vancouver Coastal Health (VCH) is recruiting a Social Worker - Case Manager for the Assertive Community Treatment (ACT) Team in Vancouver, BC. The role provides case management and clinical social work services to individuals with complex mental health, addiction and chronic medical concerns, within a client-centered and team-based care model.

You will conduct biopsychosocial assessments, provide counselling, coordinate care plans, and liaise with interdisciplinary teams and community resources
