Deliver clinical medication management at Health Sciences North using drug therapy assessment and interdisciplinary patient care. Support inpatients and outpatients across HSN services. The Pharmacist provides medication management, drug information, therapy support, and distribution services under the Clinical Manager, Pharmacy. Responsibilities include evaluating medication appropriateness, supporting treatment plans, and addressing drug-related problems with patients, families, prescribers, and healthcare professionals. The position also contributes to education, quality improvement, documentation, and safe medication practices.

Key Responsibilities
- Assess medication therapies prescribed for HSN patients
- Provide treatment recommendations through oral and written follow-up
- Complete medication reconciliation with patients and families




- Review medication orders and ensure timely distribution
- Perform therapeutic drug monitoring for high-risk medications

Requirements:
- Four-year Bachelor’s Degree in Pharmacy from an accredited university
- Current Certificate of Registration in positive standing with OCP
- Three years’ clinical Pharmacy experience preferred
- Knowledge of electronic order entry and clinical systems
