You will assess needs, create and review care plans, and support transitions by connecting clients with local services. Designated primary providers remain accountable for assigned caseloads while collaborating across care teams.

Requirements:

- College of Nurses of Ontario registration
- Passion for community care and evidence-informed practice
- Current CPR certification
- Driver's license, vehicle, and travel availability
- Willingness to attain required Saint Elizabeth certifications
