This ongoing Diabetes Specialist Nurse opportunity sits within the Countywide Diabetes Service. The Band 6/7 post provides specialist diabetes care in a nurse-led environment. You will monitor clinical needs, educate patients, and promote independence through evidence-informed self-care support.

Key Responsibilities:

- Provide specialist diabetes assessment and management
- Monitor and adjust care plans following current guidelines
- Promote patient independence and long-term self-care
- Deliver education supporting healthier lifestyle management
- Maintain detailed clinical records and professional standards

Requirements:

- Current NMC registration and strong nursing credentials
- Significant experience delivering diabetes care
