As a Field Nurse Case Manager, you will support patients throughout treatment within a Patient Support Program. Your work combines patient care, healthcare navigation, therapy coordination, and adherence support. You will collaborate with healthcare providers, pharmacies, reimbursement specialists, and internal teams to facilitate therapy access.

Key Responsibilities:

- Deliver patient-centric case management within established program protocols
- Provide disease-state, therapy, and medication education
- Coordinate care with physicians, clinics, pharmacies, and internal teams




- Conduct telephone and virtual follow-up and monitoring calls

Requirements:

- Diploma or degree in nursing
