About Avee Health Avee Health is a Canadian healthcare technology company focused on improving access to healthcare through technology-enabled medical services. We work closely with physicians and clinical teams to deliver accessible, effective, and patient-centred healthcare. As our services continue to grow, we're exploring opportunities to strengthen clinical support, improve continuity of care, and ensure timely resolution of patient-related follow-up activities.
About the Role
We're looking for a qualified and patient-focused Nurse Practitioner (NP) to support our clinical operations and work collaboratively with physicians and our Operations team. In this role, you'll help manage clinical follow-ups, address appropriate outstanding patient-care tasks, coordinate with pharmacies and other healthcare providers, and contribute to safe and timely continuity of care. You'll also play an important role in supporting clinical workflows when physicians are unavailable, on leave, or no longer practising with Avee. This is an opportunity to combine independent clinical judgment with collaborative, technology-enabled healthcare delivery in a growing organization.
Key Responsibilities
1. Clinical Follow-ups & Patient Care
- Review, assess, and manage assigned patient-related clinical follow-up tasks within your authorized scope of practice.
- Evaluate patient concerns, determine appropriate clinical actions, and arrange additional assessment or referral when required.
- Review relevant laboratory and diagnostic results, initiate appropriate follow-up actions, and communicate findings to patients when clinically appropriate.
- Prescribe, renew, or modify medications within applicable professional and regulatory requirements.
- Maintain accurate, timely, and complete clinical documentation in Avee's electronic systems. 2. Physician Support & Continuity of Care
- Collaborate with physicians to address outstanding clinical tasks and reduce delays in patient follow-up.
- Support continuity of care during physician absences or transitions through appropriately authorized clinical coverage arrangements.
- Review and resolve eligible outstanding patient matters previously assigned to unavailable or departing physicians.
- Identify clinical concerns requiring physician consultation, escalation, or additional care.
- Coordinate patient handoffs and follow-up responsibilities to maintain safe and consistent care. 3. Pharmacy & Healthcare Provider Coordination
- Respond to clinical inquiries from pharmacies regarding prescriptions, medication management, and patient care.
- Assess and address appropriate prescription clarification, renewal, or modification requests.
- Communicate with physicians, pharmacists, and other healthcare professionals to resolve patient-care issues.
- Ensure medication-related decisions and communications are documented appropriately. 4. Clinical Operations & Workflow Improvement
- Collaborate with the Operations team to improve clinical task management, follow-up processes, and patient-care coordination.
- Identify recurring clinical workflow challenges and recommend practical improvements.
- Support the development of clear procedures for task assignment, clinical escalation, documentation,
and handoffs.
- Provide clinical input on improvements to Avee's technology platform, including task queues, clinical permissions, and workflow visibility.
- Contribute to the development of safe, efficient, and scalable clinical support processes. 5. Quality, Safety & Professional Standards
- Deliver evidence-informed, patient-centred care in accordance with applicable BC regulatory requirements and professional standards.
- Exercise independent clinical judgment and recognize when consultation, referral, or escalation is appropriate.
- Maintain patient confidentiality and comply with applicable privacy and information-security requirements.
- Participate in clinical quality improvement initiatives, case reviews, and relevant professional development.
- Maintain all required skilled registrations, certifications, and competencies. 6. In-Person Patient Consultations & Clinical Care
- Provide in-person clinical consultations and assessments for patients attending Surlang Medical Clinic during scheduled on-site days.
- Conduct comprehensive patient assessments, including reviewing medical histories, evaluating presenting symptoms, performing physical examinations, and identifying clinical concerns.
- Diagnose and manage acute and chronic health conditions within the authorized NP scope of practice.
- Develop and implement individualized treatment and care plans based on clinical findings, patient needs, and evidence-informed practices.
- Order and interpret appropriate laboratory tests, diagnostic investigations, and other clinical assessments as required.
- Prescribe, renew, or adjust medications and provide patient education regarding treatment plans, medication use, and ongoing health management.
- Provide preventive healthcare services, health education, and appropriate screening based on patient needs.
- Identify patients requiring specialist consultations, additional investigations, or higher levels of care, and initiate appropriate referrals.
- Collaborate with clinic staff, physicians, and other healthcare professionals to ensure efficient patient flow and coordinated care.
- Maintain accurate, timely, and complete documentation of all patient consultations, clinical decisions, and treatment plans in the designated electronic medical record system. 7. Post-Visit Follow-ups & Patient Continuity
- Take responsibility for appropriate clinical follow-up activities arising from in-person patient consultations.
- Review and act on laboratory results, diagnostic reports, specialist recommendations, and other clinical information related to patients assessed at the clinic.
- Communicate relevant test results, treatment recommendations, and next steps to patients in a timely and clinically appropriate manner.
- Arrange follow-up consultations, reassessments, referrals, or additional investigations when required.
- Monitor patient progress and adjust treatment plans based on clinical findings, test results, and patient responses.
- Coordinate with clinic administrative staff to ensure patients receive appropriate follow-up appointments, instructions, and care coordination.
- Track and address outstanding clinical actions arising from patient visits to minimize delays and reduce the risk of missed follow-ups.
