We are seeking Registered Nurses and Registered Practical Nurses with community or home care experience to join our nursing team. About the Role
You will provide nursing care to clients with a range of needs, including recovery after hospitalization, chronic disease management, complex care, and palliative support. Assignments will reflect your nursing designation, demonstrated competencies, and the client’s care requirements.

Key Responsibilities
- Complete nursing assessments, identify care priorities, and contribute to individualized care plans.
- Administer prescribed medications and treatments, monitor responses, and report concerns promptly.
- Provide wound assessment and dressing changes, catheter care, ostomy care, and other nursing interventions within your competence.
- Provide infusion, vascular access, enteral feeding, or subcutaneous therapy support where appropriately trained and assigned.
- Support clients receiving palliative and end-of-life care with compassion and respect.
- Recognize changes in condition and communicate with the care team to arrange timely follow-up.
- Teach clients and families about care routines, symptom monitoring, and equipment use.
- Maintain accurate,



timely documentation and clear handovers.
- Follow infection prevention practices and respect client privacy, preferences, and cultural needs.
- Collaborate with Up-to-date Angels’ nursing leadership, physicians, and other care providers. Qualifications
- Current registration in good standing with the College of Nurses of Ontario as an RN or RPN.
- Previous community nursing or home care experience.
- Strong assessment, clinical judgment, organization, and communication skills.
- Ability to work independently while recognizing when to seek guidance or additional support.
- Current BCLS/CPR certification.
- Satisfactory Vulnerable Sector Check and professional references.
- Ability to travel reliably to client assignments. Additional experience is an asset: acute care, palliative care, complex wound care, IV therapy, PICC/CVAD care, subcutaneous infusions, CADD pumps, suctioning, ostomy care, or enteral feeding. Why Join Modern Angels Healthcare?
