Registered Nurse (RN) – Acute Care Experience Modern Angels Healthcare | Greater Toronto Area Bring your clinical expertise to care that makes a difference. We are seeking experienced Registered Nurses with a strong acute care background to support clients with complex care needs in their homes and community settings. The Role You will provide nursing care to clients transitioning from hospital to home, recovering from surgery, managing complex conditions, or receiving palliative and end-of-life care. This role requires strong assessment skills, confidence working independently, and the ability to recognize and respond to changes in a client’s condition.

Key Responsibilities

- Complete comprehensive nursing assessments and develop, implement, and evaluate individualized care plans.
- Administer medications and prescribed treatments safely, according to orders, professional standards, and agency policies.
- Provide IV therapy, vascular access care, wound care, catheter care, ostomy care, and enteral feeding support within your demonstrated competencies.
- Provide palliative care, symptom assessment, subcutaneous medication administration, and infusion pump care, as applicable.
- Monitor clients’ responses to treatment, identify changes promptly, and communicate concerns to the appropriate care team.
- Support hospital-to-home transitions through client assessment, education, and care coordination.
- Teach clients and families about treatments, equipment, symptom monitoring, and care routines.
- Maintain accurate,



timely documentation and explicit communication with clients, families, and healthcare professionals.
- Follow infection prevention and control practices and maintain a safe care environment. Qualifications
- Current registration in good standing as a Registered Nurse with the College of Nurses of Ontario.
- Recent acute care nursing experience, preferably in emergency, critical care, medical-surgical nursing, oncology, or a comparable specialty.
- Strong assessment, clinical judgment, prioritization, and problem-solving skills.
- Demonstrated competence in caring for clients with complex medical needs.
- Excellent communication and documentation skills.
- Ability to work independently while collaborating with a broader care team.
- Current CPR/BLS certification.
- Satisfactory background screening and ability to meet agency and client-setting health requirements. Preferred Experience
- IV medication administration and PICC/CVAD care.
- Complex wound management.
- Palliative care and symptom management.
- Subcutaneous infusions and CADD pumps.
- Respiratory care and suctioning.
