Stepping Stone is seeking an Intensive Case Manager for a one-year pilot project focused on housing stability for individuals in private market housing. You will lead individualized health and housing care plans, coordinate with a multidisciplinary team, and provide person-centred support through harm-reduction and trauma-informed practices.
You will work with a team to support clients in managing daily living, building routines, and strengthening community connections.
