Recovery Alberta seeks an experienced Social Worker II for Addiction & Mental Health Supported Living. You will work within an interdisciplinary team serving clients in transitional and long-term housing. The position includes generalist social work and collaboration across the healthcare system, requiring knowledge of community supports and the AMH service continuum.

Key Responsibilities:

- Complete psychosocial assessments for patients and families
- Develop treatment plans and coordinate overall client care
- Provide counselling and individual, group, and family treatment




- Deliver case management and transition or discharge planning
- Liaise with healthcare teams and community partners

Requirements:

- Bachelor's degree in Social Work
- Active or eligible Alberta College of Social Workers registration
- Current CPR BLS Level C and NCI certification
- Driver's license, vehicle, and appropriate business insurance
- Three to five years' community mental health experience preferred
