You will support predominantly older residents through assessment, clinical interventions, outcome evaluation, and interdisciplinary collaboration. Key Responsibilities:

- Coordinate clinical interventions and evaluate resident outcomes
- Provide information and coordinate ongoing resident care
- Support RCA and LPN work within cohesive teams
- Lead goals of care discussions with residents and families

Requirements:

- Current BCCNM registration as a Registered Nurse or Registered Psychiatric Nurse
- Approved gerontology course or equivalent education, training, and experience
- Experience supporting residents with chronic diseases
- Knowledge of aging changes, polypharmacy, and treatment protocols
- Ability to develop care plans and provide nursing interventions
