Deliver clinical medicines management as a PCN Clinical Pharmacist within East of England Community Health & Care NHS Trust’s multidisciplinary team.
Working within your clinical competence, you will provide expertise across structured medication reviews and long term condition management. The post includes supporting safe prescribing at transfer of care, implementing safer prescribing systems, and managing prescription processes. You will also handle repeat prescription reauthorisation and acute prescription requests as part of coordinated clinical medicines management.
