Provide complex home nursing care as a Registered Nurse or Registered Practical Nurse with VHA in North York, Toronto. Evening shifts include weekends.

VHA seeks three nurses for its Central Adult Visiting Program. The program serves adults aged 18 and older requiring specialized in-home clinical support. Care includes wound care, medication management, intravenous therapy, central lines, ostomy care, drain care, chemotherapy management, and mental health support.

Key Responsibilities:

- Assess, monitor, plan, and coordinate client care
- Deliver complex nursing services in clients’ homes
- Provide wound, ostomy, drain, and medication care
- Manage intravenous therapy, PICC, and central lines
- Document changes, evaluations, and care coordination requirements

Requirements:

- Registered and in valuable standing with the College of Nurses of Ontario
- Recognized RPN diploma or RN diploma or degree
- Valid First Aid or CPR certification
- Ontario G2 or G driver’s license and reliable vehicle
- Effective communication, documentation, and critical thinking skills
