Deliver community nursing care in Burk's Falls, Ontario, using patient assessments and individualized care plans during scheduled home visits. This full-time position serves patients across age groups.
As a Registered Nurse, you will care for patients with differing complexity in various settings. You will help patients remain comfortable at home while building therapeutic relationships. The position requires professional records, cooperative engagement, and required after-hours on-call coverage.
Key Responsibilities:
- Assess individualized patient needs using appropriate clinical tools
- Execute and continuously monitor individualized patient care plans
- Build rapport and trust through collaborative patient engagement
- Maintain and update professional patient encounter records

Requirements:
- Current CNO registration as a Registered Nurse in good standing
- Valid driver's license and access to a vehicle
- Community care experience considered an asset
