As an Occupational Therapist with ICS, you will assess, treat, educate, and manage clients requiring occupational therapy services. You will support hospital-to-home transitions through evidence-based interventions promoting safety, independence, and quality of life. The integrated model connects you with occupational therapists, physiotherapists, nurses, social workers, personal support workers, rehabilitation assistants, and other healthcare professionals.

Key Responsibilities:

- Complete comprehensive assessments and individualized treatment plans
- Evaluate progress and modify treatment plans




- Support protected hospital-to-home transitions
- Communicate findings and education to clients and families
- Maintain timely clinical documentation

Requirements:

- Master’s degree in Occupational Therapy or equivalent
- Registration in good standing with COTO
- Strong clinical reasoning and decision-making abilities
- Ontario driver’s license and reliable transportation
- Proficiency with electronic documentation systems
