Apply geriatric mental health expertise as a Social Worker with the Geriatric Psychiatric Service in a hybrid, full-time position.

This permanent Social Worker role supports Behavior Supports Ontario initiatives within an interdisciplinary team. You will provide client and caregiver interventions, coordinate hospital-to-home transitions, and contribute community education. The work requires clinical judgment, schedule management, and mobile service delivery across client settings.

Key Responsibilities:

- Provide intervention and support to clients, families, and caregivers
- Facilitate clients’ return home following hospitalization
- Deliver consultative education and teaching throughout the community
- Visit clientele in homes, residences, hospitals, and community settings
- Assess and prioritize requests while managing appointments

Requirements:

- Bachelor degree in Social Work
- Three years of recent geriatric population experience
- Fluent bilingual verbal and written communication
- Ontario qualified association membership in good standing
- Valid driver’s license and access to own vehicle
