CONNECT Partners in Vancouver are seeking a Service Access & Transitions Leader to bring people together, build relationships and explore possibilities. You will collaborate with Vancouver Coastal Health and partners to support thoughtful transitions into and from Cambie Gardens, helping individuals navigate change and settle into a recent chapter of life.

This is a hands-on leadership role focused on implementing CONNECT’s Life Redesign Model, working with the people we support and our coaching
